Mental Health Skills Building Criteria on a Continual Basis

The individual must demonstrate a clinical need arising form a condition due to mental, behavioral, and emotional illness that result in significant functional impairments in major life activities.

Have difficulty in establishing or maintaining normal interpersonal relationships to such a degree that they are at risk of hospitalization, homelessness, or isolation from social supports.
Exhibit difficulty in cognitive ability such that they are unable to recognize significantly inappropriate social behavior.
Require help in basic living skills such as maintaining personal hygiene, preparing food and maintaining adequate nutrition, or managing finances to such a degree that health or safety is in jeopardy.

Eligibility for Intensive In-Home (Ages 5-18)

Problems in establishing/maintaining impersonal normal relationships such that they are:

At risk for hospitalization
Out of home placement due to conflicts with family or community

Show such inappropriate behavior that repeated interventions by:

Mental Health
Social Services
Judicial System
